Complaints and Grievances

St. Raphael School has established procedures whereby students, parents, or members of the public may report SRS web content that they believe to be in violation of Section 504 of the Rehabilitation Act of 1973 and Title II of the Americans with Disabilities Act. Complaints or grievances may be submitted through the Website Accessibility Concern form below. Complaints may also be made verbally to a school administrator or may be submitted to a school administrator in writing or by email.

The ADA non-compliance complaint should include the following information:

  • Name
  • Date of complaint

Upon receiving the information, the school administrator shall immediately notify the website compliance coordinator, who will contact the reporting party within five business days and complete an investigation within 15 business days. St. Raphael School will keep a record of each complaint and investigation.
